Rules of entrance to the WIBHIS building

Before entering the building, visitors must undergo body temperature measurement and sign the statement:

I declare, that:

1. I have no symptoms indicating COVID-19 infection.
2. For the past 12 hours I have not taken medicine containing paracetamol or ibuprofenum.
3. I was not tested on COVID-19 in the last 14 days.
4. I have not been in quarantine for the last 14 days because of a suspected COVID-19 infection.
5. For the last 14 days I have not had contact with people in quarantine or isolation due to suspected COVID-19 infection.
6. For the past 14 days, to the best of my knowledge, I have not had contact with people showing signs of disease indicating COVID-19 infection.
